1. Purpose of This Confirmation
When you check the Scope of practice box during Provider registration on NoteSeek, you confirm that you understand and accept the professional obligations described in this document. This confirmation is required before you can submit your application for verification.
This document is an attestation — not a substitute for your regulatory college's standards, your professional liability obligations, or the Provider Services Agreement. It summarizes the scope-of-practice commitments that apply to every clinician on the Platform.
2. Your Professional Licence
You confirm that you are a regulated health professional licensed to practise in Ontario and that you are a member in good standing of your regulatory college. Depending on your profession, this may include:
- College of Physicians and Surgeons of Ontario (CPSO) — physicians
- College of Nurses of Ontario (CNO) — nurse practitioners and registered nurses where applicable
- College of Chiropractors of Ontario (CCO) — chiropractors
- Other Ontario colleges governing regulated health professions eligible to use the Platform
You agree to keep your licence current, comply with all college standards and policies, and notify NoteSeek promptly if your licence is suspended, revoked, restricted, or subject to conditions that affect your ability to practise on the Platform.
3. Scope of Practice
NoteSeek provides administrative medical notes — not diagnosis, treatment, or ongoing clinical care. You confirm that you will review note requests and issue notes only within the scope of practice authorized by your regulatory college and your individual licence.
Examples of scope boundaries (your specific scope depends on your profession and licence):

You may not use the Platform to issue notes outside your scope of practice, to perform acts your college has not authorized, or to issue note types the Platform has not approved for your profession.
4. Requests You Must Decline
You confirm that you will decline note requests when issuance would be outside your scope of practice or contrary to your professional obligations. You must decline requests when:
- The requested note type or clinical context is outside your licensed scope of practice.
- You lack sufficient information to make a professional judgment about the request.
- You suspect fraud, misrepresentation, or that the request is not made by or with the consent of the patient.
- The request indicates a medical emergency or mental-health crisis requiring immediate intervention (direct the patient to 911 or 988, as appropriate).
- The request asks you to diagnose a condition, prescribe treatment, or provide care the Platform is not designed to deliver.
- Your regulatory college's standards or any undertaking you are subject to would prohibit issuance.
Declining a request in accordance with this section is expected professional conduct and does not breach your obligations to NoteSeek.
5. Independent Judgment
Clinicians on the Platform exercise independent professional judgment. NoteSeek does not direct or control clinical decisions. You confirm that:
- You alone are accountable for every note you review, sign, and issue.
- NoteSeek's workflows, policies, and any AI-assisted tools do not replace your professional judgment.
- You may decline any request permitted by your scope of practice and professional obligations, without pressure from NoteSeek to issue a note.
6. Changes to Your Scope or Status
You agree to notify NoteSeek within five business days if any of the following occur:
- Your licence is suspended, revoked, restricted, or placed under conditions by your college.
- Your scope of practice changes in a way that affects the note types you may issue on the Platform.
- You change profession, college, or primary practice jurisdiction.
- You become subject to a college investigation, undertaking, or order that may affect your ability to practise on the Platform.
NoteSeek may suspend or terminate your Provider access if you fail to maintain a valid licence in good standing or if your scope no longer permits use of the Platform.
7. Acknowledgement
By checking the Scope of practice box during registration, you confirm that:
- You have read and understood this Scope of Practice Confirmation.
- The licence and scope-of-practice information you provided is accurate and complete.
- You will practise on the Platform only within your authorized scope of practice and in compliance with your college's standards.
- You understand that a false or misleading attestation may result in immediate suspension of your Provider account, invalidation of notes you issued, and referral to your regulatory college or other authorities.
